GA Completion of Duty Form – Instructions
The Graduate School requires you to fill out a GA Completion of Duty Form and have it signed by a supervisor. Instead of your specific mentor or assigned TA instructor, you should list the following people as your supervisors, depending on your assignment of GA duties:
- Dr. Lira Yoon can sign the forms for any HSP Student;
- Dr. Charissa Cheah can sign the forms for any ADP Student; and
- Dr. Angela Katenkamp Shiplet can sign the forms for Teaching Fellows
